Complete all requirements listed on the checklist and submit to missions@ccperrisvalley.org or hand deliver to missions table representative.

*Note: Deposits will not be accepted without the required application and waivers listed above. Deposit, application and waivers do not guarantee participation in the mission trip and require approval from CCPV leadership.

Mission Trip Travel Insurance: Typically we include Travel Medical Insurance in the cost of your Mission Trip, but for those trips that we do not include you may purchase insurance through Brotherhood’s Faith Venture program.  You can also purchase additional travelers insurance that is not covered by CCPV.

Delta Airlines: We are flying Delta Airlines on our Mission Trips. Review their travel policy and restrictions and sign up for a Delta Skymiles membership.  You may never find use for them but the miles can be transferred to another person.
