Mission Registration Documents:
Step 1: Short Term Mission Checklist PDF (Rev 12-6-17) See Mexico Brochure for Mexico Mission Checklist.
Step 2: Short Term Mission Application* (One time requirement. Skip this step if previously completed.)
Step 3: Medical Information & Release* (Required annually, please update if information has changed.)
Step 4: Photograph and Video Release* (One time requirement. Skip this step if previously completed.)
Step 5: Liability Waiver* (Required for each mission trip. Annually for Mexico.)
*Note: Deposits will not be accepted without the required application and waivers listed above. Deposit, application and waivers do not guarantee participation in the mission trip and require approval from CCPV leadership.
Mission Booklets & Brochures
These booklet and brochures will provide detailed information regarding passport information, health and insurance, raising support, being prepared and our code of conduct. It is essential that each short-term missionary study the applicable booklet.

Passport Information: Application documents and info, location finder, fee info.
Transportation Security Administration (TSA): Traveler, security checkpoint, screening and carry on baggage info.
Center for Disease Control (CDC): Find important vaccination and health information for country you are traveling to.
Mission Trip Travel Insurance: Typically we include Travel Medical Insurance in the cost of your Mission Trip, but for those trips that we do not include you may purchase insurance through Brotherhood’s Faith Venture program. You can also purchase additional travelers insurance that is not covered by CCPV.
Delta Airlines: We frequently fly Delta Airlines on our Mission Trips. Review their travel policy and restrictions and sign up for a Delta Skymiles membership. You may never find use for them but the miles can be transferred to another person.
